Lake County wants to be able to contact you in the event of an emergency. Please correctly enter your information and check it prior to leaving the site. If you do not enter correct information, Lake County will not be able to contact you if a public safety incident occurs. IF YOU DO NOT WISH TO BE CONTACTED in the event of an emergency, please complete the registration with the HOME PHONE NUMBER and SELECT THE "OPT OUT" box on the next page; when you submit the registration your no call option will be entered into the database. Emergency Management staff will then contact you to confirm your selection to Opt-Out of the emergency notification system.

Because of the unforseen nature of the large scale emergencies, no member of the emergency notification team can guarantee that you will receive an emergency notification for every event. Please keep informed of potential emergencies by monitoring other public safety warning and information systems such as NOAA Weather Radio, television, and other radio broadcasts.
